Health issues can turn daily life upside down. Waypoint’s programs offer assistance to individuals with a disability or chronic health condition to help navigate challenges, access resources, and maintain independence in daily lives.
Home Health Care
Waypoint is a licensed Home Health Care agency. Our Home Health Care program provides homemaking, personal care, nursing, and connections to community support services for seniors and adults with disabilities over age 18.
Health Care Coordination (HCC)
HCC (formerly Partners in Health) is a statewide, community-based support program for children and young adults with chronic health conditions and their families.

The Advocacy Department worked intently over the past two years to increase home care rates to allow Waypoint to continue providing services to elderly individuals and disable adults.
