Health Care Coordination (HCC)

HCC (formerly Partners in Health) is a statewide, community-based support program for children and young adults with chronic health conditions and their families.

A chronic health condition lasts for at least six months and cannot be prevented by vaccines, cured by medicine, or just disappear. Health conditions such as asthma, cerebral palsy, diabetes, epilepsy, and many more are considered chronic. These chronic conditions can change daily life and create challenges, both for both for an individual and the family responsible for their care.

HCC provides support for children and young adults from birth to age 21 who have health conditions expected to last a year or more, as well as to the children’s families. The program is free, regardless of family income.

  • Identify needs
  • Work directly with families to locate and access supportive resources
  • Collaborate with schools, insurance companies, and healthcare providers
  • Provide recreational and respite opportunities.
  • Create social and recreational opportunities to interact with other families who face similar concerns
  • Develop a community support network
  • Provide respite funding when needed
